Eighteen atopic asthmatic people were stratified by symptom score and compared with six atopic nonasthmatics and seven healthy nonatopic volunteers. The article confirms earlier reports of increased interleukin-4 secretion by peripheral blood cells following allergen stimulation but was important in additionally showing that increasing asthma symptom scores correlated with decreasing interferon gamma production and a decreasing interferon gamma/interleukin-4 secretion ratio. These observations, similar to those of Nurse et al. (J Allergy Clin Immunol 1997, 100:662-668), raise the important possibility that the potential for T cells to maintain interferon gamma production to offset excess interleukin-4 secretion may be a marker for being capable of controlling Th2 mediated pulmonary inflammation.

A possible role for upregulation of interleukin-9 expression in predisposing to asthma was supported by qualitative trait locus mapping in inbred unimmunized C57BL/6 (B6) mice which show decreased AHR in response to the drug atricurium compared with hyperresponsive DBA/2 (D2) mice. Interleukin-9 mRNA expression in the lungs and in splenocytes following Con A stimulation was markedly reduced in the atricurium hyporesponsive B6 mice compared with the hyperresponsive D2 mice, Interestingly, splenocytes from BDF1 mice, which are intermediate in hyperresponsiveness, demonstrated intermediate levels of interleukin-9 expression, and cloned cells demonstrated that the B6 message was significantly less abundant than were the D2 message suggesting, a cis-acting element in the B6 locus that resulted in decreased induction of the gene.

This was a very important paper showing a dominant effect of the R576 allele of interleukin-R receptor a chain on the presence of atopy as defined by either the hyper IgE syndrome, severe atopic dermatitis, levels of IgE above 95 IU/ml, or a positive radioimmunosorbent test in response to common inhalant allergens. A point mutation at amino acid residue 576 in which a glutamine was replaced by an arginine was significantly increased in affected individuals compared with nonaffected controls. As a measure of the hyperfunctioning of the mutant receptor, B cells from wild-type and homozygous or heterozygous individuals were stimulated with interleukin-4, and CD23 on their B cells was measured. The level of CD23 expression was higher on those bearing the R576 allele. Finally, regulation of signaling was altered in those with the R576 allele and associated with a change in the binding specificity of the adjacent tyrosine residue at position 575 to Stat-6 versus SHP-1. Although the presence or absence of asthma was not discussed, this article has implications for regulation of IgE production highly associated with atopic asthma.

This provocative article perhaps more than any other has focused recent epidemiologic studies and research in experimental animal models on examining the role for certain early childhood infections in protection against asthma. Japanese children who had positive PPDs that persisted until age 12 years had a lower incidence of asthma and other atopic symptoms: lower serum IgE; lower serum interleukin-4, interleukin-13, and interleukin-10; and higher serum interferon gamma than non-PPD responders or PPD responders at age 6 years who converted to a negative PPD by age 12 years. Because all children had received BCG and had equivalent exposures to Mycobacteria tuberculosis as measured by PPD responses in family members and had similar exposure to allergens and familial histories of atopy, the conclusion was that exposure to the infectious agent with a persisting Th1 response was the reason for protection against atopy and asthma.

This article describes a mouse model for asthma in which unmethylated CpG dinucleotides previously shown to enhance a Th1 response were coadministered with the immunizing antigen. This measure protected against the development of airway eosinophiia, Th2 cytokine induction, IgE production, and AHR. Furthermore, in a previously sensitized mouse, CpG oligodeoxynucleotides also prevented airway inflammation following airway challenge with the antigen. Although driving a strong Th1 response using CpG dinucleotides might have negative effects in the production of Th1-mediated autoimmune diseases, this article was important in demonstrating the possibility of using these bacterial products in therapy for atopic diseases.
