This study suggests an important correlation between the predominant activation of particular T-cell classes (CD4 versus CD8) and the type of drug reaction. Most interesting are the findings that HLA expression is increased in patients with bullous and hepatic involvement, and that CD8+ T-cell predominance correlates with the noted development of hepatic injury.

This study demonstrates that keratinocytes from patients with various cutaneous inflammatory diseases produce increased amounts of the chemokine CCL27, which can be induced by TNF-α and interleukin-1β. CCL27 then binds to CCR10 on circulating T cells, therein providing a homing mechanism for T cells to enter the cutaneous compartment.

This is the first in-vivo demonstration, in an experimental setting, that nontoxic doses of a drug can be made toxic in combination with underlying endotoxemia. The data provide important support for the assertion that concomitant infection may increase susceptibility to adverse drug events.
