In a prospective study of 673 children with ALL, the BCR-ABL fusion was detected in 3.2% of cases. Although there were no differences in presenting features, this group of patients had a higher incidence of poor responders to prednisone and a worse event-free survival rate.

The TEL-AML1 fusion was detected in 27% of childhood ALL cases and was associated with a significantly lower rate of relapse compared to TEL-AML1-negative cases (0/22 versus 16/54). TEL-AML1 formed homodimers in vitro and could also heterodimerize with the normal TEL protein.

J Clin Oncol 1997, 15:1150-1157. In a study of 188 B-precursor ALL cases, TEL gene rearrangements were detected in 48 cases (26%) and were associated with an excellent outcome (91% 5-year EFS). TEL rearrangement was shown to be a favorable prognostic factor independent of age and leukocyte count.

N Engl J Med 1997, 336:317-323. Bone marrow samples from 24 ALL patients were analyzed using quantitative PCR of immunoglobulin gene rearrangement and a clonogenic blast colony assay. Levels of minimal residual disease differed significantly between the 7 patients who relapsed and the 17 who remain in complete remission. In addition, residual PCR positivity was detected in 15 of the 17 patients in remission, up to 35 months after therapy.

In a study of 53 patients with overt CNS disease at diagnosis, intensive systemic and intrathecal chemotherapy combined with craniospinal irradiation produced excellent results, with a 5-year EFS that was similar to that for patients without CNS disease (69% versus 67%). In addition, 6 Gy was an adequate dose of spinal irradiation.
