An excellent review of parasitemia, parasite physiology, and response to treatment. The study advocates the use of a 48 h parasite reduction ratio as a prognosticator and guide to therapy. It reviews the important consideration of immunity and therapeutic response when comparing interventions in immune versus naive populations. It presents a cogent argument for the use of a treatment combination of any new, more slowly eliminated antimalarial drug with a rapidly acting antimalarial drug such as an oral artemisinin, especially if resistance to the new drug can develop through a simple point mutation.

These results in rats emphasize the need for regulatory level studies in primate models.

The first study outside China to test a drug combination (AB) against the most resistant falciparum strains. Significantly fewer side-effects (including fewer neurological side-effects) were seen in the AB-treated group. Trials continue of higher dose and longer duration regimens. The relatively low population immunity and the highly resistant falciparum strains endemic here suggest that the efficacy results from these ongoing studies on the Thai-Burmese border will be applicable world-wide.

Most importanly, 2 weeks after therapy, no children treated with CGP56697 had gametocytes versus 29% of those treated with pyrimethamine/sulfadoxine.

Earlier trials with the same antigen had failed to protect. Success was attributed to the formulation of the RTS,S antigen with a potent adjuvant system containing MPL and QS21 in an oil-in-water emulsion.
