In this study the authors show close similarities in the immunopathology between a subtype of active MS lesions and those induced experimentally by direct injection of LPS into the white matter. Patterns of microglia activation in the lesions suggest stimulation through innate immunity, involving precipitated fibrin. Tissue damage follows a pattern of hypoxic injury, most likely due to radical-mediated mitochondrial injury.

J Exp Med 2007; 204:571-582. This study expands previous reports on the role of fibrin precipitation in neurodegeneration in EAE. It provides clear evidence that fibrin-induced microglia activation can be blocked by interfering with fibrin binding to the Mac-1 receptor. This has a neuroprotective effect in EAE. Thus, fibrin-induced microglia activation can be blocked by a pathway that does not interfere with the clotting cascade

    • Chora AA, Fontoura P, Cunha A, et al. Heme oxygenase-1 and carbon monoxide suppress autoimmune neuroinflammation. J Clin Invest 2008; 117:438-447. Heme oxygenase is upregulated in EAE lesions in the course of a stress response to tissue injury. Heme-oxygenase deficient animals develop more aggressive EAE, and the disease is ameliorated when heme oxygenase-1 is induced chemically. A similar protective effect is also seen when animals are exposed to low concentrations of carbon monoxide. Thus heme oxygenase-1 appears to be one of the protective factors induced by cell stress within lesions, and stimulation of heme oxygenase-1 may have an anti-inflammatory and neuroprotective effect in diseases such as MS.

    • Black JA, Liu S, Carrithers M, et al. Exacerbation of experimental autoimmune encephalomyelitis after withdrawal of phenytoin and carbamazepine. Ann Neurol 2007; 62:21-33. It has been shown before that treatment of EAE animals with sodium channel blockers has a major protective effect. As these drugs are in use for treatment of epilepsy patients for many years without major side effects, they are very promising neuroprotective agents in MS. This study, however, shows that abrupt discontinuation of treatment in EAE animals leads to a very severe rebound of inflammation within the CNS, which is associated with high mortality of animals. This is a very important observation, which has to be considered in clinical trials of MS patients with sodium channel blockers, which have already been initiated

This study describes another very important endogenous stress protein, which plays a protective role in inflammatory demyelinating lesions. AlphaB-crystallin is highly expressed in MS lesions. The protein is neuroprotective in EAE experiments. MS patients, however, develop an immunodominant T-cell and antibody reaction to this protein. This indicates, that an important endogenous protective mechanism is inactivated in MS patients through a specific immune response.

This study describes a new molecular mechanism controlling myelination and remyelination. Blocking this pathway enhances remyelination in EAE lesions and reduces permanent axonal injury and loss. LINGO-1 is a promising new therapeutic target for stimulation of remyelination in EAE and potentially also in MS lesions.
