Improved graft survival after renal transplantation in the United States, 1988 to 1996

This retrospective analysis of the influence of demographic characteristics and transplant and post-transplant variables on graft survival for 93,934 renal transplantations performed in the US between 1988 and 1996 concludes that there has been a substantial increase in short-term and long-term survival of kidney grafts from both living and cadaveric donors.

This study showed that lymphocyte trafficking is altered by the lysophospholipid S1P and by a phosphoryl metabolite of the immunosuppressive agent FTY720, producing lymphopenia in blood and thoracic duct lymph by sequestration of lymphocytes in lymph nodes, but not spleen. Inhibition of lymphocyte recirculation by activation of S1P receptors may result in therapeutically useful immunosuppression.

The study found that FTY720 significantly prolonged skin allograft survival in a dose-dependent manner, displaying more potent immunosuppressive activity than CsA or tacrolimus in MHC-incompatible rat strains. The authors suggest that the mechanism of action of FTY720 is sequestration of circulating mature lymphocytes into PLN, MLN, and Peyer's patches.

Rat spleen cells incubated with FTY720 demonstrated features characteristic of apoptosis by electron microscopy and genomic DNA fragmentation upon agarose gel electrophoresis. The drug prolonged rat liver allograft survivals to 50 days in 10 recipients, an effect that was potentiated in combination with CsA when it was delivered to recipients of canine kidney allografts.

FTY720 also delayed or prevented disease in a CD8+ T cell-mediated diabetes model of a transgenic mouse expressing LCMV glycoprotein.

FTY720 (0.1 mg/kg) combined with CsA (10 mg/kg) synergistically prolonged Wistar King Aptekman Hokkaido skin allograft survival in MHC-incompatible F344 rats, confirming prolonged survival times and potentiation of the effects of CsA. Semiquantitative polymerase chain reaction analysis, immunohistochemical staining and flow cytometry compared the activities of cell populations in animals treated with FTY720 alone or in combination with CsA.
