Interestingly, this activity ascribed to theophylline is not shared by PDE4 inhibitors and nor is it mediated via adenosine receptor antagonism. It remains to be established whether drug targeting of this pathway will lead to better antiasthma drugs.

J Biol Chem 2002, 277:37124-37130. Another recent study providing an alternative explanation for the antiinflammatory actions attributed to theophylline. Theophylline-inhibited lipid kinase activity of the phosphoinositide (PI) 3-kinase isoforms (recombinant p85α/p110α/p110β/p85α/p100δ and 100γ) and class IA and class II PI-3 kinase activity with IC50 values between 75-800 μmol/L. The phosphorylation of protein kinase (PK) B is one consequence of the activation of PI-3 kinase. Utilizing cell based assays and in vitro tissue preparations, the potency of theophylline against phosphorylation of PKB was found to be between 100-500 μmol/L. It remains to be established whether inhibition of PI-3 kinase accounts for antiinflammatory action of theophylline in asthma, particularly as plasma concentrations often associated with antiinflammatory activity are near or below 10 μg/mL (approximately 30 μmol/L; see [31]).

J Clin Pharmacol 2002, 42:297-303. Recent study demonstrating the efficacy of a PDE4 inhibitor against exercise-induced asthma. Asthmatic subjects (n = 16) received roflumilast (500 μg per day) for 4 weeks. There was significant bronchoprotection (41%) against exercise after 4 weeks of treatment. This was also accompanied by a reduction in the release of TNF-α from mononuclear cells stimulated with LPS and confirms the view that PDE4 inhibitors demonstrate antiinflammatory activity in man.

Am J Respir Crit Care Med 2002, 165:1371-1376. Clinical study demonstrating the ability of theophylline to inhibit the recruitment and activation of inflammatory cells to the lung. Theophylline (150-300 mg twice daily) was administered to subjects with COPD for 4 weeks. The plasma levels achieved with this dosing regime were between 9-11 μg/mL. Theophylline significantly reduced the number of neutrophils recovered from sputum, and the levels of neutrophil granule products including, IL-8, myeloperoxidase, and lactoferrin. Similarly, chemotaxis of neutrophils in response to IL-8 was significantly decreased in subjects who were treated with theophylline. Together, these results illustrate the potential antiinflammatory activity of theophylline in COPD at plasma levels below those considered to be therapeutic.

One of the first studies to propose that apoptosis of human eosinophils in vitro is not mimicked by PDE4 inhibition. Hence, it might be inferred that the antiinflammatory activity attributed to theophylline is mediated by mechanisms other than PDE4 inhibition. Whilst this mechanism was not investigated in this study, recent reports of a novel mechanism [10•] may need be evaluated in this model.
