Lancet 1998, 351:550-554. This prospective study of 158 children with newly diagnosed ALL demonstrated that immunologic detection of residual leukemic cells at any point during the postremission interval is a powerful predictor of relapse. Persistent disease beyond the first 3 months of continuation therapy conferred an especially poor prognosis.

Disease-free survival and treatment-related complications were comparable between 52 children who underwent allogeneic bone marrow transplantation with HLA-matched sibling marrow and 51 who received matched unrelated donor marrow for leukemia in a single institution. The improved result with unrelated donor grafts was attributed to recent innovations in histocompatibility matching, prevention of graft-versus-host disease, and antiviral prophylaxis.

Bone marrow biopsies from 40 children with newly diagnosed ALL were stained immunohistochemically for Factor VIII to detect angiogenesis. Microvessel densities of these samples were significantly higher than those of the control subjects. Bone marrow vascularity in leukemic patients was complex with arborizing branching of microvessels, in contrast to single, straight microvessels without branching in control subjects. Urinary basic fibroblast growth factor levels also were increased in all of the 22 patients tested before treatment. This finding provides a rationale for using antiangiogenic agents in the treatment of childhood ALL.
