There has been considerable debate regarding the issue of which aspect of the AD pathology correlates most closely with cognitive deficit. Not surprisingly, the number of synapses at time of death strongly correlates [7], This paper concludes that, among features related to amyloid plaques and NFTs, the amount of deposited Aβ (both diffuse and neuritic plaque) correlates most strongly. This correlation does not speak to etiology, since the pathway from the root abnormality to neuronal death is expected to be complex and dependent on the individual. Several commentaries from experts in the field are included.

A group of 93 nuns between the ages of 75 and 93 were analyzed for signs of cognitive deficits ('probable AD'). Fifty-year-old writing samples from these nuns were independently scored for idea density and grammatical complexity. The incidence of probable AD was significantly greater in the group of nuns whose writing samples scored lower with respect to these parameters. This correlation did not seem to result from the ability of the more intelligent group to compensate for neuronal loss and mask symptoms: the extent of AD pathology also inversely correlated with the complexity of writing. This preliminary study suggests either that an endogenous factor which may increase brain plasticity may also inhibit the deposition of amyloid or that overproduction of Aβ and/or its increased deposition may begin very early in life.

The brains of early-onset AD patients from a Colombian family bearing a PS1 mutation contain sugnificantly increased amounts of Aβ1-42 deposition as compared to late-onset (sporadic) AD brains. However, the levels of deposited Aβ1-40 are indistinguishable between the two groups.

Head trauma increases the risk of developing AD later in life. The brains of head trauma patients who died soon after the accident were analyzed. About one third of patients were characterized by large amounts of diffuse amyloid deposits in the brain. In this study, it is demonstrated that, in a small sample, the amount of diffuse amyloid can be predicted by apoE genotype, with the apoE4 allele being overrepresented in the group of head trauma patients with extensive diffuse amyloid.

The yeast two-hybrid assay was utilized to discover proteins which interact with the cytoplasmic loop of PS1. The interacting protein was identified as δ-catenin, a brain-specific member of the catenin family. The catenins are homologous to the products of the Drosophila segment polarity gene Armadillo, a component of the Wingless signalling pathway. The implication is that the PS proteins may also be involved in development and that the partially elucidated pathways that control development may also play a role in AD; for example, the activity of PS1 could be modified by activation of a cell-surface receptor.

This paper reports that the 100 amino acid carboxy-terminal fragment of APP polymerizes in vitro to form amyloid fibrils. This fragment, called C100, forms intracellular amyloid fibrils when overexpressed in COS cells [53]; the phenotype is 'rescued' by coexpression of apoE [48], These fibrils were treated with the nonspecific protease, proteinase K, to afford a protease-resistant fragment which is indistinguishable from synthetic Aβ1-40 on SDS-PAGE. Whether this cleavage product is Aβ1-40, Aβ1-42, another uncommon isoform (e.g. Aβ1-41), or a mixture of these was not determined, but is a critical question, since the authors propose that this in vitro proteolysis may mimic the biological mechanism used to generate Aβ. The phenomenon of a protein polymer being specifically cleaved by a sequence-nonspecific protease is precedented in the case of the scrapie-specific form of the prion protein (PrPSc), which is characterized by the fact that a specific amino-terminal sequence is cleaved by proteinase K. Another possibly related finding that is also reminiscent of the prion characteristics is the fact that Aβ1-42 fibrils can be internalized by cultured cells and induce abnormal stability in an APP fragment [54].
