    • 16 Heckers S, Rauch SL, Goff D, Savage CR, Schacter DL, Fischman AJ, Alpert NM. Impaired recruitment of the hippocampus during conscious recollection in schizophrenia. Nature Neurosci 1998; 1:318-323. This report is of special interest because it suggests the existence of complex abnormal interactions between brain regions during performance of a memory retrieval task. Using PET, in the brains of patients with schizophrenia it was found that a prefrontal overactivation coincides with a hippocampal underactivation.

Using a cluster analysis after a single photon emission computed tomography study on 40 patients with schizophrenia, the authors found that delusional patients had hypoperfusion in bilateral frontal, superior temporal and mesial temporal regions of the left hemisphere. The authors suggest the existence of a functional disconnection between frontal and temporomesial/limbic structures as a likely anatomic basis for delusional disorders.

Proc Natl Acad Sci U S A 1998; 95:11406-11411. Using technically refined methodologies, deformations in the hippocampus were detected, which were found to be localized to subregions sending projections to the prefrontal cortex. The authors suggest that it might be more informative to look for changes in the shape of a structure than for volume comparisons.

Patients with average onset of schizophrenia at about 10 years were subjected to MRI scans of their brains and rescans with identical apparatus and methods after 2 years. Compared with control individuals, patients with schizophrenia showed significantly greater reductions of several temporal lobe structures, suggesting a progressive reduction with ongoing illness in childhood-onset schizophrenia.

Using conventional MRI, this study stresses altered connectivity patterns between thalamic nuclei and prefrontal areas as a possible cause for delusional symptomatology. As other reports reviewed here, it demonstrates that it might be necessary to look at different brain regions simultaneously and consider their mutual interdependence when interpreting probably deviant patterns, both on the neural and on the behavioural side.

Technical issues relevant to the interpretation of image data are stressed as well. The roles of prefrontal, orbitofrontal and amygdaloid structures in depressive illnesses are thoroughly discussed.

The necessity is stressed for studying the social and biological determinants of behaviour in order to establish a common framework, for example, by pointing to relationships between inherited and acquired mental illnesses. Furthermore, the perspective of a possible renaissance of psychoanalytic thought is outlined. Such a perspective has also been given in some of the articles on stress-related mental deteriorations (see also [78-80,88]).
