Termed P-Rex1, this enzyme has Rac-GEF activity that is activated directly by PIP3 and by Gβγs, both independently and synergistically. P-Rex1 appears to be particularly adapted to function downstream of heterotrimeric G proteins in neutrophils, and antisense inhibition of expression in a neutrophil-like cell line decreased C5a-induced superoxide production.

This article provides a detailed description of the clinical features of an infant with recurrent, rapidly progressive soft tissue infections who had a variety of neutrophil function defects similar to those reported for Rac2 knockout mice. The finding prompted analysis of the Rac2 gene in this patient, leading to identification of a point mutation in the Rac2 coding sequence that results in expression of a dominant-negative form of Rac2. The previous functional and molecular studies are also summarized in this article.
