This is an interesting meta-analysis of all randomized placebo-controlled clinical trials of the tolerability and efficacy measured in terms of 50% seizure reduction of the new AEDs gabapentin, lamotrigine, tiagabine, topiramate. vigabatrin and zonisamide in patients with refractory partial epilepsy. No difference was established between the drugs.

This is an interesting discussion of the role and shortcomings of meta-analysis of controlled trials of AEDs using 50% seizure reduction as the measure of efficacy. It is argued that new AED trials can determine appropriate use of AEDs but fail to determine their exact role in treating patients. The need for large multicenter studies in well defined populations with well characterized epilepsy syndromes and aetiologies, using seizure freedom as the primary measure of efficacy is emphasized.
