Five adolescents developed a fever, rash, and arthralgias/arthritis after taking minocycline for 10 to 30 days; this reaction was typical of symptoms associated with serum sickness-like reaction. Four patients were receiving minocycline for the treatment of acne and one patient received the drug for pneumonia.

The authors of this study identified 17 cases of hypersensitivity syndrome reaction, 7 cases of serum sickness-like reaction, and 25 cases of drug-induced lupus in their review of two databases and the literature. Serum sickness-like reactions and hypersensitivity syndrome reactions due to minocycline occurred within approximately 4 weeks of initiation of therapy, whereas drug-induced lupus occurred on average 2 years after the start of therapy.

Eleven cases of minocycline-induced systemic lupus erythematosus, 16 cases of hepatitis, and seven additional cases are reviewed. Two patients died while taking the drug for acne and a third required a liver transplant. In five of the cases, rechallenge resulted in recurrence of the drug reaction.

Mitchell P: Paediatric lamotrigine use hit by rash reports. Lancet 1997, 349:1080. Lamotrigine has been associated with severe skin rashes in children. The risk in children may be as high as 1 in 50, and the risk in adults is probably closer to 1 in 1000. Additional risk factors include simultaneous use of valproic acid, exceeding the recommended initial lamotrigine dose, or a too-rapid increase in dose.

A 10-month-old girl with biliary atresia was diagnosed with latex allergy. While in the hospital, the patient received a latex-containing pacifier. Within 1 hour, wheezing and upper airway obstruction led to respiratory failure. The patient fully recovered. Although latex allergy is not a new phenomenon in children, this case emphasizes that many sources of latex antigens are present in the hospital.

In a recent report, five children infected with HIV were desensitized to TMP-SMX using a rapid, 4-hour, oral protocol. Three children successully completed desensitization and started maintenance therapy; however, the other two children developed reactions during the desensitization. One patient developed localized pruritus that progressed despite treatment with diphenhydramine. In another patient, headache, shaking chills, and a fever developed 2 hours after completion of desensitization. TMP-SMX was discontinued. Two months later the patient was given dapsone and developed a similar reaction.
