Bethesda, Maryland: Fogarty International Center, National Institutes of Health; August 2003. Working Paper No. 11. In this in-depth review, the authors provide an overview of morbidity and mortality patterns in sub-Saharan Africa. Recent evidence is re-analysed to provide estimates of the direct health consequences of malaria infection in Africa and to examine the indirect contribution of malaria infection on mortality, other cause morbidity and disability.

Prevention of increasing rates of treatment failure by combining sulfadoxine-pyrimethamine with artesunate or amodiaquine for the sequential treatment of malaria. J Infect Dis 2003; 188:1231-1238. This study investigates response to treatment and the prevalence of resistance-conferring mutations in recrudescent infections in children treated with sulfadoxine-pyrimethamine, sulphadoxine-pyrimethamine plus amodiaquine, or sulphadoxine-pyrimethamine plus artesunate. Despite frequent recrudescence in the sulphadoxine-pyrimethamine group, failure in the sulphadoxine-pyrimethamine plus amodioquine group was uncommon, suggesting that it may prolong the therapeutic life span of available treatment regimens.

    • Taylor TE, Fu WJ, Carr RA, et al. Differentiating the pathologies of cerebral malaria by postmortem parasite counts. Nat Med 2004; 10:143-5. This autopsy report challenges the clinical picture of severe malaria in that it demonstrates that although a number of children were considered to have clinical features of severe malaria, up to 25% died of other causes. The authors suggest that this has implications for clinical trials and immunogenetic studies designed to examine malaria-specific disease associations.

This is a retrospective analysis of clinical correlates with fatal outcome in children admitted with severe malaria. Many of the correlates associated with a fatal outcome would be regarded as evidence for hypovolaemia in critically-ill children, irrespective of cause.

    • Polyuria in association with Plasmodium falciparum malaria in a region of unstable transmission
    • Grimwade K, French N, Mthembu D, Gilks C. Polyuria in association with Plasmodium falciparum malaria in a region of unstable transmission. Trans R Soc Trop Med Hyg 2004; 98:255-260. In this retrospective study of 175 adult cases of severe malaria no significant differences were found between the polyuric and non-polyuric cases at presentation and mortality in both groups was similar. This contrasts sharply with areas in the world where transmission of malaria is unstable and adults develop severe disease, for whom acute renal failure with ollguria is relatively common and is a major contributor to mortality.
