The review concludes that life-long adherence to a moderate, low fat, high carbohydrate diet, with pharmacological support, when necessary, currently offers the best chance of helping obese individuals to lose weight and reduce their risk of other life-threatening conditions (e.g. cardiovascular disease).

J Am Diet Assoc 1996; 96:1245-1253. An interesting article identifying nine different strategies for reducing intake of dietary fat. Regression analysis indicated that the fat reduction strategy most responsible for decreasing participants' intake of dietary fat was decreased use of fat as a flavouring (e.g. decreased use of salad dressings, spreads on bread and vegetables and gravy).

A long-term study on the effect of spontaneous consumption of reduced fat products as part of the normal diet on indicators of health

1) than the full fat products and if the full fat group consumed 70% of their total fat intake from the foods provided by the study. Subjects in the reduced fat group were, therefore, required to consume at least a minimum amount of the reduced fat products. The only methodological drawback to this study is that subjects who failed to eat the minimum amount of reduced fat products were told so by the investigator (and withdrawn from the study if they failed to comply during 4 consecutive weeks). Thus, this may have interfered with the free-living concept of the study. References [20] and [21] report on different aspects of the results from this same study.

This study points out that although consumer trials hold potentially high ecologic validity they may be criticised for low experimental control and, particularly, for their dependence on self-reported data. In this study, for example, reported energy intakes decreased over the trial and were inconsistent with the lack of change in body weight. The authors conclude that consumer trials would benefit from the development of more reliable methods of measuring habitual eating behaviour.

An interesting article aimed at investigating how diabetic patients integrate foods containing tat substitutes into their diet. The article provides a good general overview of the scientific literature on fat substitutes (including sections on health and nutrition benefits of fat replacers and on energy and fat compensation). A useful table is included that illustrates the main categories of fat replacers, trade names within each category, examples of how they are identified on food labels, the types of foods they are used in, their role in foods and calorie content and their regulatory status (GRAS or food additive approval).
