Separate associations between visceral and subcutaneous adipose tissue distribution, insulin and glucose levels in obese women

Ross R, Fortier L, Hudson R. Separate associations between visceral and subcutaneous adipose tissue distribution, insulin and glucose levels in obese women. Diabetes Care 1996; 19:1404-1411. Forty obese women had total IAF and total body subcutaneous fat measured by multislice MRI. In addition, IAF was subdivided into intraperitoneal and retroperitoneal fat volume. Correlation with glucose metabolism (oral glucose tolerance test) showed that IAF had the strongest correlation with insulin resistance after controlling for the influence of whole body subcutaneous fat. In addition, the metabolic correlations were not improved by segmenting IAF into intra-peritoneal and retroperitoneal adipose tissue.

J Clin Endocrinol Metabol 1997; 82:383-387. This Finnish group identified 50 identical twins out of a registry of 1500 pairs who were discordant for obesity by a mean of 18 kg and performed an oral glucose tolerance test. Body fat distribution was measured by MRI. In genetically identical subjects, the intrapair differences in insulin sensitivity were inversely correlated with IAF but not subcutaneous fat or total body fat.

An MRI chemical shift technique was validated by liver biopsy in 19 subjects. MRI liver fat index accounted for 76% of the variance biopsy fat. This MRI liver fat index was then measured before and after weight loss. Liver fat correlated close with baseline IAF and changes in liver fat correlated closely with reductions in IAF.

Sixty-seven patients with familial combined hyperlipidaemia with a mean body mass index of 27 were compared with a healthly control group matched for age and body mass index. Patients with familial combined hyperlipidemia had higher insulin and glucose AUC after an oral glucose tolerance test than control individuals. This finding was present in non-obese and obese patients. In patients with a body mass index <27, 25-68% were hyperinsulinaemic compared with 0-4% in control individuals.

Three hundred and sixty-six adults from 100 nuclear families underwent abdominal computed tomography and underwater weighing. Adjusting for age, 70% of the variance in total abdominal fat could be explained by familial factors. Heritability estimates, adjusted for age and total fat, were 56% for IAF and 46% for abdominal subcutaneous fat.

In 86 non-obese Australian female twins, DEXA was used to measure central abdominal fat and non-abdominal fat. A strong genetic influence (approximately 70% of the population variance) was demonstrated on central abdominal fat both before and after adjustment for genetic influences on total body fat. This suggests the existence of genes specific for abdominal obesity.
