Renal outcomes with different fixed-dose combination therapies in patients with hypertension at high risk for cardiovascular events (ACCOMPLISH): A prespecified secondary analysis of a randomized controlled trial

10.1016/S0140-6736(09)62100-0 1:CAS:528:DC%2BC3cXktlGitr8%3D 20170948 This is recent evidence demonstrating greater renal protection by combining a blocker of the RAS with a DHP CCB, compared with combining a blocker of the RAS with a thiazide diuretic in hypertensive patients. This finding suggests that once the potentially detrimental effect of afferent arteriolar vasodilation by DHPs is counteracted by blockade of the RAS, the pleiotropic effects of DHPs [37•] are unmasked

Intracellular angiotensin II production in diabetic rats is correlated with cardiomyocyte apoptosis, oxidative stress, and cardiac fibrosis
-
10.2337/db08-0805 1:CAS:528:DC%2BD1MXhsleitrs%3D 18829990 Work in this laboratory has unequivocally documented activation of an intracellular RAS in experimental diabetes. They show here that increased generation of cardiomyocyte Ang II cannot be blocked by either an ACE inhibitor or an ARB but is effectively blocked by the direct renin inhibitor aliskiren, which was able to suppress superoxide production and fibrosis. This observation is important because it suggests that newer modalities in the blockade of the RAS may be more effective in preventing Ang II-induced insulin resistance, diabetes, or diabetic target organ damage
