Although lipase concentrations were significantly lower in patients with chronic pancreatitis and pancreatic cancer compared with people with a normal pancreas, they were not accurate enough for routine clinical use (PPV, 73%; negative predictive value, 85%).

Further, the authors failed to explain how clearly discernible pathologic differences in some chronic pancreatitis groups (eg, endophlebitis in TCP) and clear differences in natural histories of etiologically distinct forms of chronic pancreatitis support their hypothesis that a common immunologic change underlies all causes of chronic pancreatitis.

By applying strict criteria, the investigators found, compared with previous publications, a higher frequency of mutations of R122H, N29I, or A16V, greater disease penetrance, and a higher lifetime pancreatic cancer risk, and found that the onset of pancreatic cancer occurred 14 years earlier in smokers. In addition, in contrast with previous studies, the common R122H mutation was not associated with more aggressive disease, but these people sought treatment 4 to 5 years earlier than people with N29I or no identifiable mutations and were more frequently hospitalized than the N29I patients. Patients with N29I mutations, however, were treated at an earlier age with pancreatic resection for pain.
