Antigen-presenting cells have developed receptors that bind the sugar mannan, which is most often found on yeasts. This group has shown that coupling mannan to a protein antigen (GST-MUC1 [variable number tandem repeat] fusion protein) enables the generation of strong MUC1-specific cytotoxic T-lymphocytes (CTLs) responses in mice at low doses and with no adjuvant. Moreover, whether a Th1 or Th2 response is generated can be manipulated by how the mannan is attached to the glutathione S-transferase protein.

Patients with metastatic breast cancer were immunized several times with a highly attenuated vaccinia virus (MVA) engineered to express both MUC1 and interleukin-2. Two partial responses were observed. One patient with several liver metastases had shrinkage of tumors such that they could be removed surgically. More than 5 years later, the patient is alive and tumor free.
