This retrospective analysis of 711 adult patients with IgA nephropathy from Glasgow, UK; Helsinki, Finland; Sydney, Australia; and Toronto, Canada found a geographical variability in renal survival and the slope of creatinine clearance. When age, creatinine clearance, proteinuria, and mean arterial blood pressure at clinical presentation were taken into account, the variability related mostly to leadtime bias and the inclusion of milder cases at centers with apparently good outcomes.

In an effort to identify IgA nephropathy patients without a renal biopsy, the authors found that 82% of patients with IgA nephropathy versus 35% of patients with other primary renal diseases had at least three of the following laboratory findings: serum IgA/C3 ratio greater than 3.01, more than five red blood cells per high-power field on urinalysis, persistent proteinuria greater than 0.3 g/day, and a serum IgA level greater than 315 mg/dl.

These findings strengthen the rationale for early treatment with ACE inhibitors and angiotensin II type 1 receptor blockers, even in the absence of hypertension or severe proteinuria.

Improved renal survival was strongly associated with a greater reduction in proteinuria.
