    • Tschumperlin DJ, Dai G, Maly IV, et al. Mechanotransduction through growth-factor shedding into the extracellular space. Nature 2004; 429:83-86. Recent evidence has shown that mechanical stress may directly lead to remodelling. Here mechanical stresses modelled to occur in asthmatics in vivo were shown to shrink the lateral intercellular space surrounding epithelial cells. This triggered cellular signalling via autocrine binding of epidermal growth factor family ligands to the epidermal growth factor receptor.

This study showed that reticular basement membrane thickening is already present in children with difficult asthma and to a similar extent to that seen in adults. In addition, there was no association with age, symptom duration, lung function, or eosinophilic airway inflammation. Airway remodelling may be a very early finding in asthma.

The mechanisms are unknown but this may indicate that airway remodelling is reversible.
