Blood 1997, 90:58-63. This article shows that treatment with rhTPO enhanced platelet recovery in myelosuppressed primates. As would have been predicted from the studies of myelosuppressed mice treated with thrombopoietin, rhTPO also stimulated erythropoiesis in vivo. However, the effect of rhTPO on erythropoiesis in the primates was curbed by the development of functional iron deficiency.

Multiple classes of progenitor cells were mobilized into the blood. The half-life of rhTPO in vivo was long: 20 to 30 hours.

This randomized double-bund placebo-controlled dose escalation trial of PEG-rHuMGDF in patients undergoing chemotherapy for lung cancer showed that administration of PEG-rHuMGDF raised the nadir platelet count and hastened platelet recovery to baseline levels in comparison to the placebo-treated patients.

Patients with solid tumors being treated with a myelosuppressive chemotherapy regimen were randomly selected to receive PEG-rHuMGDF plus granulocyte colony-stimulating factor, or placebo plus granulocyte colony-stimulating factor. The addition of PEG-rHuMGDF accelerated platelet recovery to baseline values, and increased mobilization of primitive hematopoietic progenitor cells into the blood.

The numbers of CFU-GM were expanded more than 2 million fold during the 6-month culture period. If it can be demonstrated that stem cells are also expanded, this approach may eventually prove useful for cord blood transplantation programs.

Blood 1997, 89:2679-2688. Human C034+ peripheral blood progenitor cells were cultured in the presence of a cocktail of cytokines including thrombopoietin, then infused into patients undergoing autologous peripheral blood stem cell transplantation. The infusion of the ex vivo-generated megakaryocytes was safe. Whether this approach will decrease the need for platelet transfusions remains to be determined.
