Pediatrics 1998, in press. An abnormal signal intensity in the posterior limb of the internal capsule on magnetic resonance imaging between days 1 and 17 was found to be an extremely accurate predictor of neurodevelopmental outcome at 1 year of age in 73 term infants with hypoxic-ischemic encephalopathy.

In this population-based case control study, maternal fever exceeding 38°C in labor was associated with increased risk of unexplained cerebral palsy (odds ratio, 9.3) as was a clinical diagnosis of chorioamnionitis. Maternal fever was also linked with low Apgar scores, need for resuscitation and neonatal seizures.

Pediatrics 1998, 101:185-193. The effect of high-dose allopurinol (40 mg/kg) was assessed on cerebral perfusion, free radical formation and electrical brain activity in 22 severely asphyxiated newborns. Cerebral perfusion was investigated using near infrared spectroscopy, free radical formation by plasma non-protein-bound iron, antioxidant capacity and malondialdehyde and electrical activity by the cerebral function monitor. This pilot study suggested a benefit in these factors without toxic side-effects.

It is demonstrated that genetic disruption of PARP provides profound protection against glutamate-NO-mediated ischemic insults in vitro and major decreases in infarct volume after reversible middle cerebral artery occlusion in mice. These results give evidence for a primary involvement of PARP activation in neuronal damage following focal ischemia.

    • Marion DW, Penrod LE, Kelsey SF, Obrist WD, Kochanek PM, Palmer AM, et al.: Treatment of traumatic brain injury with moderate hypothermia. N Engl J Med 1997, 336:540-546. In a randomized controlled trial the authors compared the effects of moderate hypothermia and normothermia on 82 patients with severe closed head injuries. The patients assigned to hypothermia were cooled to 33°C at a mean of 10 hours after the injury, kept at 32°C to 33°C for 24 hours and then rewarmed. Hypothermia did not improve the outcome of patients with coma scores of 3 or 4 on admission; however, in those with coma scores of 5 to 7 on admission, hypothermia hastened neurologic recovery and may have improved the outcome
