Interesting paper showing that when assessed in biopsy-proven myocarditis CMR has lower sensitivity, even using Lake Louise criteria, than that reported in ref. 19 according to the type of clinical presentation, being lower in heart failure and in arrhythmic than in infarct-like presentations. Thus, according to the authors' findings a negative CMR does not always rule out biopsy-proven acute myocarditis and should not preclude the use of endomyocardial biopsy in clinically suspected cases. This observation is in keeping with ESC 2013 Task Force recommendations set in ref. 3.

First prospective follow-up study in relatives of dilated cardiomyopathy patients showing that a positive serum cardiac autoantibody test at baseline predicts development of dilated cardiomyopathy at 5-year follow-up in subjects with and without minor echocardiographic abnormalities at baseline. This study shows that immune-mediated dilated cardiomyopathy has a long pre-clinical phase and that the autoimmune serology test provides an non-invasive marker of pre-clinical disease.
