This article reviews the clinical effects of isolated a1-adrenergic stimulation, highlighting the specific instances in which there is an overall clinical benefit. These include critical aortic stenosis, tetralogy of Fallot and hypotension during spinal anaesthesia for caesarean delivery in which there is a favourable regional oxygen supply to demand ratio, which outweighs its global effects.

This study demonstrates the efficacy of relatively high-dose, fixed rate, prophylactic phenylephrine infusions at preventing maternal nausea and hypotension, but their use was associated with reactive hypertension. It concludes that variable rate infusions should be investigated.

However, it did make the interesting observation that neonates who were homozygous at codon 16 to arginine appeared to be resistant to ephedrine-induced fetal acidosis.
