J Child Adolesc Psychopharmacol 2002; 12:11-25. This study found five symptoms that distinguish pediatric BP from ADHD. It incisively addressed the diagnostic confusion that may result from reliance on the nonspecific symptom of irritability.

This paper provides a fascinating account of the history of childhood BP from descriptions of ancient Greek physicians to 1980. An enjoyable, must-read that shows the evolution of current thought.

14. Craney JL, Geller B. A prepubertal and early adolescent bipolar disorder-I phenotype: review of phenomenology and longitudinal course. Bipolar Disord 2003; 5:243-256. This review summarizes their phenomenological and outcome data to date. These authors also compared pediatric versus adult BP and discussed the need for community physicians to be better educated about recognizing prepubertal BP.

Topics covered range from SSRIs to light therapy and exploratory adult treatments such as repetitive transcranial magnetic stimulation and vagal nerve stimulation.

These authors provided a thorough and readable review of a critically important topic. The discussions raise the hypothesis of a critical period for susceptibility to environmental insult on neuroendocrine systems implicated in depression and anxiety.
51. Sheline YI, Gado MH, Kraemer HC. Untreated depression and hippocampal volume loss. Am J Psychiatry 2003; 160:1516-1518. These investigators found evidence for structural brain changes resulting from untreated depression. This is an important work when pondering risks and benefits of treating childhood MDD.
