This study evaluated MMP-9 levels in sputum during and after spontaneous attacks of asthma. Pro-MMP-9 activities and MMP-9 concentrations in asthmatic patients significantly exceeded those of control subjects. The activities of pro-MMP-9 were significantly higher in acute asthmatic patients than in stable asthmatic patients. The elevated MMP- 9 activities significantly decreased after 7 and 28 days of therapy.

The authors ultimately concluded that epithelial activation by IL-13 or IL-4 plays a critical role in initiating remodeling through release of TGF·beta2. TGF-β2 then activates the underlying myofibroblasts to secrete matrix proteins and smooth muscle and vascular mitogens to propagate remodeling changes into the submucosa.

Am J Respir Crit Care Med 2001, 163:517-523. Study on the underlying pathology of mucus production in asthma using bronchial biopsies and induced sputum. Even mild asthma was associated with goblet cell hyperplasia and increased stored mucin in the airway epithelium, whereas moderate asthma was associated with increased stored mucin and secreted mucin. The authors suggested that chronic degranulation of goblet cells may contribute to chronic airway narrowing in moderate asthma.
