J Clin Endocrinol Metab 1999, 84:899-905. Leptin increased with age before puberty in both sexes. With the onset of puberty, leptin levels continued to increase in girls but decreased in boys. Girls gained more fat mass than boys, whereas boys gained more fat-free mass. There were no significant differences in the regression coefficients between the sexes for leptin and fat mass, allowing for the effect of the fat-free mass. Leptin levels correlated significantly with a large number of other hormones, but none was independent of changes in body composition.

Leptin concentration was nearly threefold higher in postpubertal than in prepubertal girls, but was relatively similar in pre- and post-pubertal boys. Significant sex differences in leptin concentration existed in prepubertal, pubertal and post-pubertal children. After adjusting for total body fat, fat-free mass, and age, testosterone concentration was negatively associated with leptin levels in pubertal boys, while estradiol concentration was positively associated with leptin level in girls.

J Clin Endocrinol Metab 1998, 83:3469-3475. Obese children had similar estradiol levels and equivalent bone ages at a younger chronologic age than nonobese children. Leptin was higher in these obese children but did not correlate with estradiol level or bone age. The same pubertal stage was reached by the nonobese children at a much lower leptin concentration and fat mass, but at slightly later chronologic age.

    • Functional hypothalamic amenorrhoea: A partial and reversible gonadotrophin deficiency of nutritional origin
    • 51 Couzinet B, Young J, Brailly S, Le Bouc Y, Chanson P, Schaison G: Functional hypothalamic amenorrhoea: a partial and reversible gonadotrophin deficiency of nutritional origin. Clin Endocrinol 1999, 50:229-235. The fat body mass and leptin are lower and the lean body mass higher in patients with functional hypothalamic amenorrhea than in control subjects. Fat restriction is able to interfere with gonadotrophin secretion. Assessment of nutritional markers allows recognition of mild nutritional insufficiency as a common cause of functional hypothalamic amenorrhea. The gonadotrophin deficiency is partial and may be reversible after improvement of nutritional intake and body composition.

    • Human leptin deficiency caused by a missense mutation: Multiple endocrine defects, decreased sympathetic tone, and immune system dysfunction indicate new targets for leptin action, greater central than peripheral resistance to the effects of leptin, and spontaneous correction of leptin-mediated defects
    • 58 Ozata M, Ozdemir IC, Licinio J: Human leptin deficiency caused by a missense mutation: multiple endocrine defects, decreased sympathetic tone, and immune system dysfunction indicate new targets for leptin action, greater central than peripheral resistance to the effects of leptin, and spontaneous correction of leptin-mediated defects. J Clin Endocrinol Metab 1999, 84:3686-3695. Leptin deficiency was associated with multiple endocrine abnormalities. Humans who survive the negative effects of leptin deficiency during childhood can, in contrast to ob/ob mice, over decades compensate some of the effects of leptin deficiency on immunity and endocrine function including hypogonadism.

    • Effects of recombinant leptin therapy in a child with congenital leptin deficiency
    • 59 Farooqi IS, Jebb SA, Langmack G, Lawrence E, Cheetham CH, Prentice AM, et al.: Effects of recombinant leptin therapy in a child with congenital leptin deficiency. N Engl J Med 1999, 341:879-884. This is the first report of long-term leptin treatment in a 9-year-old girl with congenital leptin deficiency and severe obesity. At baseline, she weighed 94 kg, and she lost 16 kg of fat during 12 months of leptin administration through a decrease in apetite and energy intake. Gonadotropin concentrations increased slowly.
