J Clin Endocrinol Metab 1999, 84:899-905. Leptin increased with age before puberty in both sexes. With the onset of puberty, leptin levels continued to increase in girls but decreased in boys. Girls gained more fat mass than boys, whereas boys gained more fat-free mass. There were no significant differences in the regression coefficients between the sexes for leptin and fat mass, allowing for the effect of the fat-free mass. Leptin levels correlated significantly with a large number of other hormones, but none was independent of changes in body composition.

Significant sex differences in leptin concentration existed in prepubertal, pubertal and post-pubertal children. After adjusting for total body fat, fat-free mass, and age, testosterone concentration was negatively associated with leptin levels in pubertal boys, while estradiol concentration was positively associated with leptin level in girls.

Fat restriction is able to interfere with gonadotrophin secretion. Assessment of nutritional markers allows recognition of mild nutritional insufficiency as a common cause of functional hypothalamic amenorrhea. The gonadotrophin deficiency is partial and may be reversible after improvement of nutritional intake and body composition.
