J Clin Endocrinol Metab 1999, 84(11):1-8. T cells within the adrenal gland have direct cell-cell contact with epithelial cells of the adrenal zona reticularis. This provides a mechanism for immune system-mediated stimulation of androgen secretion in vitro and helps explain how impaired T cell function results in decreased androgen levels in vivo. Also, it provides evidence for a non-ACTH-mediated mechanism of adrenocortical androgen regulation. These findings may provide an evidence-based strategy for DHEA treatment in some disorders of the immune system. Patients with systemic lupus erythematosus, rheumatoid arthritis, and AIDS have low DHEA levels and might benefit from such treatment.

Findings indicate that both TNF-α and IL-1 participate in mediating some of the behavioral effects of infection with Mycoplasma fermentans, whereas only TNF-α, but not IL-1, is involved in mediating M. fermentans-induced adrenocortical activation. Suggests that cytokines within the brain are involved in mediating at least some of the neurobehavioral and neuroendocrine abnormalities that may be produced by M. fermentans in AIDS patients.

Cellular levels of dehydroepiandrosterone sulfotransferase (DST) mRNA were increased in a dose-dependent fashion in response to ACTH; DST mRNA was less responsive to ACTH stimulation than was 17 hydroxylase (CYP 17) mRNA. The stimulatory effects of ACTH on DST mRNA levels were blunted by both TGF-beta and TNF-alpha; the inhibitory effects of TNF-α on DST mRNA were more striking than were those on CYP 17 mRNA. These data suggest that DST production can be altered by several agents acting on the DST gene.

The hormonal mediators of a stress-induced enhancement of skin immunity are identified. Adrenalectomy, which eliminates the glucocorticoid and epinephrine stress response, eliminated the stress-induced enhancement of skin delayed-type hypersensitivity (DTH). Low-dose corticosterone or epinephrine administration significantly enhanced skin DTH and produced a significant increase in the number of T cells in lymph nodes draining the site of the DTH reaction. In contrast, high-dose corticosterone, chronic corticosterone, or low-dose dexamethasone administration significantly suppressed skin DTH. These results suggest a role for adrenal stress hormones as endogenous immunoenhancing agents, and show that hormones released during an acute stress response may help prepare the immune system for potential challenges for which stress perception by the brain may serve as an early warning signal.

TNF-α may be important in modulating the attenuated pituitary adrenocortical response following 4-day endotoxin treatment, and IL-1-β appears to play a prominent role in attenuating the response following 8-day endotoxin treatment. The effects of cytokines on the HPA axis may be mediated at all levels, including the brain and pituitary gland, as well as having direct effects at the adrenal level.

The differential patterns observed between pituitary-dependent and adrenal-dependent groups are compatible with a positive immunomodulatory role of pituitary pro-opiomelanocortin-derived peptides that effectively counterbalance, at least partially, glucocorticoid immunosuppression.

Malignancy is associated with diffuse enlargement of the adrenal glands and resistance to dexamethasone-induced suppression of the HPA axis, which is not due to ectopic ACTH secretion.
