This interesting study demonstrates that the administration of the insulin-sensitizing agent troglitazone to women with PCOS and impaired glucose tolerance results in a marked attenuation of hyperinsulinemia that is associated with improvements in insulin secretion, ovarian androgen biosynthesis, and enhanced fibrinolytic system capacity. These results are consistent with the hypothesis that insulin modulates the 17α-hydroxylase and 17,20-lyase activities of ovarian cytochrome P450c17α, further clarifying the pathogenetic role of insulin in PCOS, and suggest that troglitazone may be useful as an adjunct therapy.

This article stresses that the state of insulin resistance characteristic of puberty is most evident at Tanner stage II, and is coincidental with the onset of gonadal secretion, a dramatic increase in IGF-I levels, and a marked decrease in sex hormone-binding globulin concentrations, thus supporting the established relationship among these parameters.

This study demonstrates that hyperinsulinemia and decreased IGF binding protein-1 and sex hormone-binding globulin concentrations are already present before puberty and throughout all stages of pubertal development in girls with a history of premature pubarche, a cohort with a well-known increased incidence of ovarian hyperandrogenism. These data suggest a common pathogenetic link between hyperinsulinemia and adrenal-ovarian androgen excess and points toward a peripubertal onset of these disorders.

This interesting review examines the role of increased serine phosphorylation of adrenal and ovarian cytochrome P450c17α and increased serine phosphorylation of the insulin receptor in the pathogenesis of the three principal features of PCOS. According to this theory, adrenal hyperandrogenism and hyperinsulinemia would be present before the pubertal period, and would herald the development of ovarian hyperandrogenism when the activation of the hypothalamic-pituitary-gonadal axis occurs.
