A household survey was carried out with a representative sample of 10 600 Australians aged 18 years or over. Participants were interviewed with the Composite International Diagnostic Interview. Unlike national surveys in the UK and USA, this one did not exclude the elderly. People aged 65 years or older were found to have the lowest prevalence of anxiety, affective and substance use disorders. This was true both for men and for women.
6. Erkinjuntti T, Østbye T, Steenhuis R, Hachinksi V. The effect of different diagnostic criteria on the prevalence of dementia. N Engl J Med 1997; 337:1667-1674. Over 1800 participants in the Canadian Study of Health and Aging were diagnosed for dementia according to six commonly used systems. The proportion with dementia varied from 3% (with International Classification of Diseases-10 criteria) to 29% (with Diagnostic and Staistical Manual of Diseases, volume 3). Only 20 persons were diagnosed as demented by all six systems. These findings show that there is no 'true' prevalence of dementia; the prevalence rate depends on the definition adopted.

A meta-analysis of apolipoprotein E genotype as a risk factor for Alzheimer's disease was carried out using data from 40 research groups, giving a total of 5930 Alzheimer's disease patients and 8607 control individuals. Risk of Alzheimer's disease was increased for genotypes a2/ a4, a3/a4, and especially a4/a4, whereas it was decreased for genotypes a 2/a2 and a2/a3. The effects were found across racial groups, but tended to be weaker for African-Americans and Hispanics, but stronger in Japanese. The increased risk with an a4 allele was found at all ages between 40 and 90 years, but the risk decreased after age 70 years.
14. Bullido MJ, Artiga MJ, Recuero M, Sastre I, Garcia MA, Aldudo J, et al. A polymorphism in the regulatory region of APOE associated with risk for Alzheimer's disease. Nature Genet 1998; 18:69-71. This study is not strictly epidemiology, but it nevertheless presents findings on a potentially important new genetic risk factor. These researchers have extended work on the apoE gene to look at new polymorphisms in the transcriptional regulatory region of the gene. One of these polymorphisms was associated with Alzheimer's disease in two clinical samples. Individuals homozygous for a common allele were found to be at increased risk and this association was independent of a4 allele status. The authors suggest that the polymorphism may affect risk for Alzheimer's disease by altering the amount of apoE protein produced by the gene.

This is a prospective study on risk of dementia, with over 5000 participants who were nondemented at baseline. Food intake was assessed at baseline by a food-frequency questionnaire and dementia was assessed 2 years later. Risk of VaD was associated with higher intake of total and saturated fat. Risk of Alzheimer's disease was associated with lower fish consumption (relative risk of 0.3). The importance of this study is that so little is known about nutritional risk factors for Alzheimer's disease and the findings potentially have great public health impact. However, the findings need replication before being taken too seriously.
33. Itzhaki RF, Lin W, Shang D, Wilcock GK, Faragher B, Jamieson GA. Herpes simplex virus type 1 in brain and risk of Alzheimer's disease. Lancet 1997; 349:241-244. This study examined DNA from the brains for 46 AD patients and 44 elderly control individuals. The apoE a4 allele was found to increase risk for AD marginally, whereas the presence of DNA from the herpes simplex virus type 1 was not a risk factor. However, the risk was much higher in individuals with both the a4 allele and DNA from the herpes simplex virus type 1. The authors hypothesized that reactivation of the virus is more damaging in carriers of the a4 allele. This study illustrates a trend for epidemiological studies to examine synergistic effects of the apoE a4 allele and environmental risk factors.

Lancet 349:151-154. Atherosclerosis was investigated in participants in a large population-based study; 284 participants were demented and 1698 were not. Atherosclerosis was assessed in the carotid arteries (measured by ultrasonography) and in the large vessels of the legs (measured by the ratio of the ankle-to-brachial systolic blood pressure). All indicators of atherosclerosis were associated with both VaD and AD. The association was particularly strong in individuals who carried the apoE a4 allele. The importance of this study is that it challenges the distinction between AD and VaD. To some extent, AD may also be a vascular dementia.
37. Dent OF, Sulway MR, Broe GA, Creasey H, Kos SC, Jorm AF, et al. Alcohol consumption and cognitive performance in a random sample of Australian soldiers who served in the second world war. Br Med J 1997; 314:1655-1657. Daily alcohol intake was assessed in over 200 ex-servicemen (mean age 64 years). Eighty-five per cent of the men were drinkers, with 19% of these drinking at the 'hazardous' level and 21% at the 'harmful' level. An extensive battery of neuropsychological tests and a computed tomography scan were given 9 years later, but no associations were found with alcohol consumption. Furthermore, mortality over the 9-year follow-up did not relate to alcohol consumption. This study questions the conventional wisdom that alcohol abuse is an important risk factor for dementia.

This paper reviews the evidence for differences in prevalence of depressive disorders and depressive symptoms across the life span. It also examines life-span differences in exposure to different classes of risk factors. Risk factors are grouped into the following classes: biological vulnerability, stressful life events and psychological diathesis. The review provides a comprehensive coverage of the American literature, but omits some key references from other countries.
