Urinary free cortisone and the assessment of 11beta-hydroxysteroid dehydrogenase activity in man
-
Palermo M, Shackleton CHL, Mantero F, Stewart PM: Urinary free cortisone and the assessment of 11beta-hydroxysteroid dehydrogenase activity in man. Clin Endocrinol (Oxf) 1996, 45:605-611. This paper, along with that of Best and Walker (Clin Endocrinol [Oxf] 1997, 47:231-236), illustrates that measurement of urinary free cortisol-to-cortisone concentrations may measure renal 11β-hydroxysteroid dehydrogenase type 2 activity independently of the influence of 11β-hydroxysteroid dehydrogenase type 1 activity in other tissues, both in congenital deficiency syndromes (the present article) and after pharmacologic inhibition (Best and Walker). Given the importance of cortisol metabolism in dictating tissue sensitivity to cortisol, a key step will be to improve our ability to distinguish tissue-specific enzyme activities in more detail.

Nelson's syndrome associated with a somatic frame shift mutation in the glucocorticoid receptor gene

Karl M, Von Wichert G, Kempter E, Katz DA, Reincke M, Monig H, Ali IU, Stratakis CA, Oldfield EH, Chrousos GP, Schulte HM: Nelson's syndrome associated with a somatic frame shift mutation in the glucocorticoid receptor gene. J Clin Endocrinol Metab 1996, 81:124-129. Raises the important possibility that mutations in the glucocorticoid receptor underlie the pathogenesis of Cushing's syndrome, but this hypothesis has not been supported in other studies (see Dahia et al. Increased vasoconstrictor sensitivity to glucocorticoids in essential hypertension
-
Walker BR, Best R, Skackleton CHL, Padfield PL, Edwards CRW: Increased vasoconstrictor sensitivity to glucocorticoids in essential hypertension. Hypertension 1996, 27:190-196. Both this article and that of Walker et al. (Hypertension 1998, in press) show that subjects who are sensitive to glucocorticoids in the skin (using the skin blanching assay) are predisposed to hypertension, insulin resistance, and glucose intolerance, independently of cortisol secretion and other cardiovascular risk factors. These data provide a novel paradigm to explain the association among multiple cardiovascular risk factors, invoking enhanced tissue sensitivity to cortisol.

Neuroendocrinology 1996, 64:412-418. This paper and that of Phillips et al. (J Clin Endocrinol Metab 1998, 83:757-760) show that in both rats and humans, low birth weight can be associated with higher circulating glucocorticoid levels in adulthood. In rats, this association may be explained by programming of negative feedback glucocorticoid sensitivity.
