In a large prospective case control study of women taking postmenopausal hormone therapy (The Nurses=Health Study), deaths caused by coronary heart disease and stroke were reduced relative to non users (relative risk 0.47 and 0.68, respectively), as was total mortality (0.63). After 10 years of hormone therapy the benefits for coronary heart disease were retained while the benefit for total mortality was lost because of an increase in breast cancer.

Whereas coronary arteries from animals treated with conjugated equine estrogen dilated in response to acetylcholine, tamoxifen-treated animals constricted. Thus, tamoxifen unlike other estrogens does not promote vasodilation.

The effect of raloxifene on atherosclerosis was studied in cholesterol-fed rabbits. Raloxifene reduced atherosclerosis in one segment of the aorta by one-third, compared with a two-thirds reduction with 17-β estradiol. Thus, raloxifene has only a weak effect on atherosclerosis development at the concentration used.

The effect of raloxifene and conjugated equine estrogen on experimental atherosclerosis was studied in 117 ovariectomized female cynomolgus monkeys. There was no effect of two doses of raloxifene on coronary artery atherosclerosis, while conjugated equine estrogen reduced atherosclerosis by 70%. This suggests that even though raloxifene reduces plasma cholesterol concentrations, it does not possess the direct protective effects of estrogen on the arterial wall.

Arterioscler Thromb Vasc Biol 1997; 17:2524-2531. This is the first published study to evaluate the effect of soy protein and soy phytoestrogens on experimental atherosclerosis. Male cynomolgus monkeys consuming diets containing soy protein had lower plasma cholesterol concentrations and less coronary artery atherosclerosis than did animals consuming the same soy diet in which the phytoestrogens had been extracted from the soy protein with alcohol. This suggests that the phytoestrogens were partly responsible for these effects.

There was no effect of isoflavones on plasma lipoproteins, but isoflavone treatment enhanced vasodilatory capacity in response to acetylcholine.

Arterioscler Thromb Vasc Biol 1997; 17:217-221. Conjugated equine estrogen given to cynomolgus monkeys fed an atherogenic diet for 30 months reduced coronary artery atherosclerosis by 72%. Inclusion of the progestin medroxyprogesterone acetate with the conjugated equine estrogen for the duration of the experiment completely prevented the protective effects of conjugated equine estrogen. This could have serious implications for the long-term benefits on coronary heart disease of current formulations for hormone replacement therapy which use medroxyprogesterone acetate.

Norethindrone acetate is a 19-nortestosterone derivative that reduces the uterotropic effects of estrogens and is used in combination with estrogens for hormone replacement therapy. Some progestins used for this purpose have been reported to reduce the beneficial effects of estrogen on atherosclerosis; see [64]. When norethindrone acetate was tested in this study in ovariectomized cholesterol-fed rabbits it was found to enhance the protective effects of estradiol.

After 4.1 years of treatment with conjugated equine estrogens plus medroxyprogesterone acetate in postmenopausal women with coronary heart disease, there was no reduction in the overall rates of coronary heart disease events. Since this is in contrast to other studies with estrogen alone or in combination with other progestins, one interpretation that is consistent with studies in monkeys [64] is the specific progestin (medroxyprogesterone) used in this study prevented the protective effect of estrogens.
