The relationship between cardiovascular disease and post-menopausal hormone therapy was evaluated after a 16 year follow-up in 59 337 women from the Nurses' Health Study who were 30 55 years of age at baseline. Women who took estrogen with progestin compared with women who did not take hormone therapy, had a relative risk ratio of 0.39, which was equivalent to estrogen therapy alone. Thus, addition of progestin does not attenuate the cardioprotective effects of post-menopausal estrogen therapy.

Biochim Biophys Acta 1996, 1300:210-218. The effect of 17β-estradiol was studied in the J774 mouse macrophage cell line that had been loaded with cholesteryl esters by incubation with rabbit β-VLDL. Neutral cholesteryl ester hydrolase activity was enhanced by estrogen resulting in reduced accumulation of cholesteryl esters in macrophages. This appeared to be estrogen receptor mediated because 17 α-estradiol was without effect.

Cynomolgus monkeys were given an atherogenic diet plus conjugated equine estrogen for 12 weeks at which time radiolabeled LDL was injected, and accumulation and degradation in the aorta, iliac and coronary arteries were measured. Even though conjugated equine estrogen areas reduced coronary artery atherosclerosis in a parallel group of cynomolgus monkeys by 72% (Adams MR et al. Arterioscler Thromb Vasc Biol 1997:217-221) it had no effect on LDL accumulation or degradation. In this study, after 12 weeks of cholesterol feeding there was little intimal thickening, whereas in our previous study [13], after 16 weeks of cholesterol feeding in which estrogen reduced LDL accumulation and degradation, both intimal thickening and macrophage foam cells were present. These results are consistent with the conclusion that macrophage foam cells are a target for estrogens in mediating lipoprotein accumulation and degradation in the arterial wall.
