This is the first published report describing the presence in SSc patients of PDGFR-stimulating antibodies, which can increase synthetic activity of fibroblasts and promote their differentiation into myofibroblasts. This is of great interest, because PDGF function has been linked to aberrant tissue fibrogenesis as well as intimal hyperplasia in different obliterative vascular disorders. If confirmed in larger studies, these data may provide the rationale for treatment strategies targeted to direct pathogenetic mechanisms in SSc.

Arthritis Rheum 2006, 54:963-973. This study shows that topo-1 can be recognized directly on the surface of fibroblasts by anti-topo-1 antibodies and thus activate monocytes. These findings may suggest a direct role of topo-1-specific immune response in the initiation and/or amplification of the local inflammatory and fibrotic damage in SSc.

Different AIF-1 isoforms are induced by polarized cytokines such as TGF-β or IFN-γ. AIF-1 proinflammatory properties and its ability to promote vascular smooth muscle cells migration/proliferation may be relevant in the pathogenesis of SSc vasculopathy.

This limited study describes a new small-ex vivo assay for detection of autoantigen-specific CD8+ T cells in SSc patients. This single-cell-based approach presents several important advantages: rapid ex-vivo application, testing of target tissue-specific specimens (eg, BAL fluid), and possible correlation of quantitative and functional data with relevant SSc clinical features.
