This paper first describes that HLA-DRB1 SE as well as smoking are risk factors only for the anticitrulline antibody positive subset of RA and not for the anticitrulline antibody negative subset. An etiologic model for RA is proposed, indicating that smoking induces citrullination as well as activation of immunity to citrullinated proteins, that this immunity can trigger arthritis.
