Arthritis Rheum 48:2741-2749. This case-control study in northern Sweden could show that individuals before the onset of RA had for anti-CCP 34%, IgG-RF 17%, and IgM-RF 19%. Anti-CCP antibody and IgA-RF were found to be significant predictors of RA. The presence of anti-CCP and IgA-RF predicted the development of RA, with anti-CCP antibody having the highest predictive value. Moreover, citrullination of human tissue could be identified and suggests that the production of anti-CCP can be an early autoimmunization in RA.

This study investigated the presence of anticitrulline antibodies, PAD enzymes, and citrullinated antigens in mouse models of both acute and chronic destructive arthritis. Of note, PAD4 mRNA was readily transcribed and translated by polymorphonuclear neutrophils infiltrating the synovial tissue during inflammation but not in healthy control mice. The authors concluded that citrullination of synovial antigens is apparently an active process during joint inflammation in both mice and humans, allowing the induction of anti-CCP autoantibodies in patients with RA.

This is one of the largest studies analyzing the development of lymphoma secondary to RA. The authors identified an increased frequency of diffuse large B cell lymphoma in 22 of 33 NHL patients (67%) in this RA cohort. Most interestingly, 20 of 22 patients with diffuse large B cell lymphoma had high inflammatory activity, but only 6 had been treated with a disease-modifying antirheumatic drug for more than 1 year, indicating a possible association with RA disease activity.
