In a cross-sectional study of 241 patients with temporal lobe epilepsy, volume reduction of the amygdala was detected in 19% of patients with reduced hippocampal volumes, and was associated with complex febrile seizures and intracranial infections. Both hippocampal and amygdala volume loss appeared to be mild in patients with short durations of epilepsy, and was greater in patients with longer epilepsy duration.

In a cross-sectional analysis of 27 patients with unilateral temporal lobe epilepsy undergoing evaluation for surgery, entorhinal volumes ipsilateral to the seizure focus were reduced by 13-17% in around 52% of the patients. Volume reductions of the entorhinal, perirhinal, and temporopolar cortices strongly correlated with the hippocampal atrophy.

Relationships among hippocampal atrophy detected by MRI, alterations in NAA/Cr detected by MRS, and relative neuronal and glial cell numbers were prospectively assessed in 33 patients with intractable epilepsy undergoing evaluation for surgery. There was a correlation between hippocampal volume loss and reduction in the neuronal/glial ratio in hippocampal pyramidal areas, but not in the dentate gyrus. There was no correlation of neuronal/glial cell count ratios with magnetic resonance spectroscopy measures of NAA/Cr.

The effects of seizures on memory function and neuronal populations of the hippocampus were assessed by behavioral and stereological methods following a range of three to more than 134 repeated brief seizures evoked by kindling stimulation of the olfactory bulb. Memory dysfunction was observed after about 30 kindled secondary generalized seizures, and increased as a function of the number of evoked seizures, in association with neuronal loss in the temporal hippocampus in a pattern resembling hippocampal sclerosis.

Seven days of once daily low-intensity electroconvulsive shock evoked seizures conferred resistance to neural degeneration induced by kainic acid in entorhinal cortex, perirhinal cortex, and hippocampus, as assessed histologically by reduced silver staining, prevention of DNA fragmentation associated with apoptosis, and decreases in the death associated protein BCL-xs compared with control rats also treated with kainic acid.
