In this State of the Art, more than 200 papers about the role of oxidative stress in COPD are reviewed. This can be considered as a reference paper. From this review it becomes obvious that the balance between oxidants and antioxidants is disturbed in smokers. It remains unclear, however, why only certain subjects develop COPD.

Healthy smokers have smaller decrements in lung function and fewer symptoms in response to ozone than do nonsmokers. As a possible explanation, the authors suggest that the airways of smokers might be less responsive to irritants in general, or that ozone might reduce the ozone responsiveness.

Although no induction was noted in type I epithelial cells, fibroblasts, and Clara cells, type II epithelial cells increased their manganese SOD and showed resistance against repeated ozone exposure.
