In humans following cholecystectomy and choledocholithotomy for cholesterol gallstones, experimentally induced reductions in plasma LDL and VLDL cholesterol resulted in a average 31% decrease in billiary bile salt rates without affecting cholesterol secretion rates. This finding lends strong support to the hypothesis that apolipoprotein B-containing lipoproteins are a key source of cholesterol for bile salt synthesis.

Hepatology 1999; 29:1541-1548. The unesterified plant sterol, sitostanol, was used a surrogate marker for unesterified cholesterol in HDL. Isolated rat livers were perfused with reconstituted HDL and biliary and hepatic lipids were analysed. A model is presented for transport of unesterified cholesterol from HDL into bile exclusively through the hepatocyte plasma membrane, without hepatocellular internalization of cholesterol prior to biliary secretion.

A critical role for sister-P-glycoprotein in canalicular bile salt secretion is suggested by its liver-specific expression, immunolocalization and transport kinetics and substrate selectivity of recombinant protein, which closely mimicked purified rat canalicular membrane vesicles.

[25**] that sister-P-glycoprotein is the major bile salt export pump in mammalian liver.

Ultrarapid cryofixation of livers in situ was utilized to examine billiary lipid Secretion in mdr2(+/+), mdr2(+/-) and mdt2(-/·) mice as well as a strain of mdr2(-/-) mice which was rescued by genomic insertion of the human MDR3 gene. The authors demonstrated good correlation between numbers of billiary vesicles and expression of the mdr2/MDR3 P-glycoprotein. These results strongly support a model for billiary secretion in which translocation of phospholipids across the canalicular membrane facilitates vesiculation of the ectoplasmic surface of the membrane by bile salts within bile canaliculi.
