Both genotypes were identified in AIDS patients. Interestingly, virulence studies suggest that the human genotype isolated from AIDS patients appears to be less virulent in tissue culture models.

These studies correlation the histopathology with symptoms for AIDS-related cryptosporidiosis. They document a high rate of co-infection, especially with cytomegalovirus. Severity of illness correlated with the extent and intensity of C. parvum infection only in those not co-infected with other enteric pathogens.

This small pilot study carefully documents the response to open-label therapy with the combination of paromomycin and azithromycin in AIDS-related cryptosporidiosis. The results suggest a significant clinical and parasitologic response.

A double-'blind' placebo-controlled study of nitazoxanide in the treatment of cryptosporidial diarrhoea in AIDS patients in Mexico. Trans R Soc Trop Med Hyg 1998; 92:663-666. This placebo-controlled trial documents a significant-response to treatment with nitazoxanide. The response rates, however, were not different on intent-to-treat analysis. Furthermore, the group most needing therapy (those with CD4 counts below 50) did not respond. Further studies are needed to clarify the effectiveness of this agent.
