The elucidation of the temporal roles of tumor necrosis factor-α (TNF-α), interleukin (1L)-1β, IL-1 receptor antagonist (ra), and IL-8 in the development of pulmonary inflammation following lipopolysaccharide (LPS)-induced lung injury in mice uncovered a chain of events which fit existing in vitro results. Not surprisingly, TNF-a was the first cytokine whose levels became elevated, followed shortly thereafter by concomitant increases in IL-1β and IL-8 levels and neutrophilic infiltration. Interestingly, the ratio of IL-1ra to IL-1β dropped at the onset of inflammation and later recovered and ultimately exceeded resting levels during resolution of the inflammatory process. The balance between these two factors may serve as a signal, with excessive levels of IL-1ra leading to a downregulation of inflammatory events, thereby preventing destructive inflammation. Production kinetics suggest that TNF-α triggers the release of IL-1β and IL-8 which act to enhance neutrophil recruitment and activation.

According to results obtained from a rabbit lung injury model intended to reproduce acute respiratory distress syndrome, IL-8 activity is critical for the development of inflammatory findings. Systemic lipopolysaccharide (LPS) challenge following intratracheal instillation of heat-killed streptococci led to elevated IL-8 levels and severe pulmonary inflammation. Pathologic changes were almost completely inhibited by pretreatment with anti-IL-8 antibodies. Clearly, in this model, IL-8 activity occupies a critical position in the chain of inflammatory events.

Interestingly, the enzyme's ability to induce human alveolar macrophages to produce tumor necrosis factor -α, IL-1β, and IL-8 is not dependent on its enzymatic activity; rather, a direct interaction with host cells has been postulated as the signal.

Lipopolysaccharide and CD14 were not involved. The bacterial signal is transduced through a tyrosine kinase activity, leading to the activation of the transcription factors NF-κB and AP-1 and their promotion of IL-8 gene expression.

    • Cowdery JS, Chace JH, Yi A-K, Krieg AM: Bacterial DNA induces NK cells to produce IFN-γ in vivo and increases the toxicity of lipopolysaccharides. J Immunol 1996, 156:4570-4575. Treating mice with intravenous E. coli DNA and a sublethal dose of lipopolysaccharide (LPS) resulted in significant increases in peak levels of cytokines and in mortality. Apparently, the bacterial DNA potentiates the effects of LPS. The authors suggest that priming of the system, with increased levels of interferon-γ from natural killer cells in response to bacterial DNA, accounts for the difference. This is another example of how bacterial DNA can act as a co-stimulatory molecule and alter the immune response.

A role for bacterial DNA in pulmonary inflammation was demonstrated by the intratracheal instillation in mice of unmethylated CpG motif containing bacterial DNA and oligonucleotides. Lavage fluid from the lower respiratory tract of these animals contained significantly elevated levels of cellular infiltrate and proinflammatory cytokines as compared with controls. The authors suggest relevance to human disease by showing that bacterial DNA isolated from the sputum of cystic fibrosis patients yields similar results.

The in vivo systemic effects of bacterial DNA and CpG motif containing synthetic oligonucleotides were explored following intraperitoneal injection in mice. Experiments utilizing high concentrations of DNA were similar in kinetics and magnitude to experiments utilizing lipopolysaccharide administration, including, under certain conditions, the development of lethal shock secondary to tumor necrosis factor-α production. The data suggest a possible role of bacterial DNA in septic shock.
