Emphasis is placed on drawing conclusions from prospective, randomized trials, with lesser weight given to retrospective reviews and observational data. There is good information on the nutrition assessment as well as evaluation of clinical situations in which TPN is often used.

JPEN J Parenter Enteral Nutr 1997, 21:14-20. This is a prospective, randomized trial to compare enteral with parenteral feeding in acute pancreatitis. Isonitrogenous, isocaloric formulae were used, and the enteral feed was delivered directly to the jejunum. No adverse effects were seen with the tube feeding, whereas glycemia and Ranson scores were improved, hence the conclusion that TPN need not be a knee-jerk reaction in this clinical situation.

Am J Clin Nutr 1997, 66:546-550. This important randomized, double-blind study compared hypocaloric with a more traditional TPN formula in hospitalized, obese patients. Nitrogen balance was not impaired whereas glucose control was improved in the underfed group. Protein intake was equivalent in both groups, but calories were limited to 22 kcal/kg IBW in the experimental group, compared to 22 kcal/kg actual body weight in controls subjects.

The startling result from this randomized, double-blind, controlled trial was that short-term administration of glutamine as one component of TPN in an ICU population led to a reduction in 6-month mortality. The population comprised 84 critically ill persons, and randomization was well done, giving equal APACHE scores in each group at baseline.

Patients with upper gastrointestinal malignancy were randomly assigned to enteral or parenteral feeding in the postoperative period and monitored for 30 days. The noteworthy result is that if there is isocaloric delivery of nutrients, TPN is no more likely than enteral feeding to be associated with failure of gut barrier function as assessed by a lactose-mannitol test.
