Nature 1997, 387:903-908. An important paper that moves beyond correlational analyses to show that leptin has metabolic effects in humans. Two cousins (one male, one female) with severe obesity are congenitally leptin deficient owing to a homozygous frameshift mutation in the leptin gene. Both children had normal birth weights but rapidly became obese; they are over 50% body fat. In contrast to the stunting of ob/ob mice, the children are at the 75th centile for height. The children are always hungry and eat more than their siblings, but their body temperature is normal. Also in contrast to ob/ob mice, neither child has elevated cortisol or glucose concentrations. The influence on the pituitary-gonadal axis is difficult to assess because of their prepubertal development.
