A replication-defective adenovirus vector was constructed that encodes the lacZ gene under transcriptional control of the smooth muscle cell-specific SM22α promotor (AdSM22-lacZ). After intra-arterial or intravenous transfection, lacZ was expressed only in smooth muscle cells. The ability to restrict transgene expression to certain cell types may help to limit toxicity of gene therapy.

Circulation 1997, 96:1899-1905. Adenoviral constructs encoding either amino-terminal-truncated (Rb56) or full-length retinoblastoma (Rb 110) were used to infect balloon-injured rat carotid arteries. Transfer was highly efficient as demonstrated by immunoblot analysis. After 14 days, neointima formation was reduced by 40% using either Rb form. These data corroborate an earlier study by Chang et al. [47] and confirm the usefulness of Rb as an inhibitor of smooth muscle cell replication in animal model [5].

Gene expression was efficiently achieved in endothelial cells but limited m the smooth muscle cell layer. Expression was maximal at 3 days but was fairly rapidly attenuated. This study and others [65] illustrate the difficulty in whole-vessel transfection when the endothelium is intact.

Purified fibroblast growth factor (FGF)-1 (aFGF) 0.01 mg/kg body weight was injected in close proximity to the left anterior descending coronary artery distal anastomosis in 20 patients undergoing coronary revascularization. At follow-up angiography, new capillary formation was observed around the site of injection in all patients.
