Again, a poor correlation with circulating virus load was noted. This is a challenge to countries such as the UK, where currently less than 20% of HIV-infected mothers are being identified during pregnancy.

An ambitious study of almost 7000 mothers in Malawi, which assessed whether cleansing the birth canal during labour with 0.25% chlorhexidine might reduce the transmission of HIV. It did not, except in the subgroup whose membranes were ruptured for over 4 h. Postpartum infections in the mothers were, however, lower, and neonatal sepsis was reduced.

Sixty-nine per cent had some signs or symptoms by 1 year, but 72% were surviving longer than 4 years. This analysis did not take into account the wide variations in approaches to treatment in the different centres.
