Obstetric complications in autism: Consequences or causes of the condition?
-
Bolton PF, Murphy M, Macdonald H, Whitlock B, Pickles A, Rutter M: Obstetric complications in autism: consequences or causes of the condition? J Am Acad Child Adolesc Psychiatry 1997, 36:272-281. Analysing data from a case-control family study, obstetric complications were found to be more frequent in autistic probands after controlling for maternal age and birth order. This was due to an increase in mild complications that are unlikely to play a direct causal role. Obstetric adversities relate to familial loading for the 'lesser variant, of autism and it was concluded that autism and obstetric adversity probably share the same (genetic) risk factors.

Lainhart JE, Piven J, Wzorek M, Landa R, Santangelo SL, Coon H, Folstein SE: Macrocephaly in children and adults with autism. J Am Acad Child Adolesc Psychiatry 1997, 36:282-290. In a series of 91 autistic subjects, the rate of macrocephaly (head circumference >97th percentile) was found to be 14%. In some cases the age of onset of macrocephaly appeared to be in the school age years. Macrocephaly was unassociated with seizure disorder, nonverbal IQ or neurological signs.

Effects of tryptophan depletion in drug-free adults with autistic disorder
-
McDougle CJ, Naylor ST, Cohen DJ, Aghajanian GK, Heninger GR. Price LH: Effects of tryptophan depletion in drug-free adults with autistic disorder. Arch Gen Psychiatry 1996, 53:993-1000. Among autistic adults, tryptophan depletion produced a significant symptom exacerbation in two-thirds of the sample, with an increase in sensory-motor behaviours and anxiousness; there was no change in social relatedness and repetitive thoughts and behaviour.

A double-blind, placebo-controlled study of fluvoxamine in adults with autistic disorder
-
McDougle CJ, Naylor ST, Cohen DJ, Volkmar FR, Heninger GR, Price LH: A double-blind, placebo-controlled study of fluvoxamine in adults with autistic disorder. Arch Gen Psychiatry 1996, 53: 1001-1008. In a double-blind placebo-controlled study with 30 autistic adults, fluvoxamine (up to 300 mg/day) was significantly better than placebo in reducing repetitive behaviour and thoughts, agression, maladaptive behaviour and in improving the social use of language. Most of the beneficial effects were seen in week 4 and sustained in week 12. Tolerance was excellent.
