Of these patients. 37% developed HNSCC, whereas only 6% of patients without LOH developed HNSCC. In a population with a 50% prior probability of recurrent or metachronous tumor. microsatellite analysis of 9p21 and 3p14 would give a positive predictive value of about 86%. The high rate of telomerase activity in even premalignant oral lesions makes it an attractive target for screening; however, the low rate of detection and high false-negative rate in oral rinses limits its usefulness. The false-negative results may be partially related to salivary RNAse activity. Rinses were also positive in 6% of control patients.

Oncogene 1996, 12:1663-1668. Mutation in the p53 gene or HPV-16 DNA is frequent in oral squamous cell carcinoma (40%). and HPV infection as well as mutation appears to be frequent causes for p53 inactivation in development of HNSCCs. Also described is a poorer prognosis for patients with tumors that were both p53 mutation-negative and HPV-negative.

N Engl J Med 1995, 332:429-435. Molecular analysis of p53 mutations seems to allow detection of microscopically undetectable tumor cells in surgical margins and lymph nodes from HNSCC patients following resection. Appears to predict patients at a substantially increased risk of local recurrence.

    • Field JK, Kiaris H, Risk JM, Tsiriyotis C, Adamson R, Zoumpourlis JR, Woolgar J, Vaughan ED, Spandidsos DA, Jones AS: Allelotype of squamous cell carcinoma of the head and neck: fractional allele loss correlates with survival. Br J Cancer 1995, 72:1180-1188. Allelic loss was quantitated as the fractional allele loss (number of chromosome arms on which allele loss is noted). Fractional allele loss above the mean value of 0.22 correlates with local metastasis and decreased survival. This method initiates attempts to measure the risk of progressive HNSCC disease with progressive chromosomal loss.

Cancer Res 1995, 55:5030-5037. Enumerates the chromosomal abnormalities associated with HNSCC metastatic disease to lymph nodes. Overexpression of p53 protein also correlated well with an increased incidence of chromosomal imbalances and also qualified as a secondary risk factor for metastatic disease.

Cancer Res 1995, 55:975-978. Cyclin D1 is thought to be important in activating retinoblastoma-repressed cell cycle transcriptional factors. Overexpression of cyclin D1 is associated with cell proliferation and here is also associated with aggressive HNSCC tumors resulting in decreased disease-free interval and survival.
