Two thousand seven hundred and thirty-one men in this longitudinal cohort study (averge age 79) were followed for 3.5 years for risk of falls/fractures. Men with less than 33 min/day of moderate activity had a 70% greater risk of fracture, and those over 80 had a 43% increase in falls, extending physical activity guidelines to the oldest old for these indications.

Seven hundred sixty-seven men and women average 79 from the Framingham Study were assessed for mobility and body composition using dual energy X-ray absorptiometry. Low lean mass was associated with self-reported mobility limitations after accounting for body size and fat, and lean and fat mass had independent effects on mobility in elders, suggesting that sarcopenia definitions should consider body size and fat.

Thirteen trials were included in the review and 11 in the meta-analysis. The extended exercise program showed modest effect sizes for knee extension strength, balance, and some physical performance-based tests. Community-based programs had larger effect sizes compared with home-based programs. This is the first meta-analysis to provide evidence that an extended exercise rehabilitation program for patients with hip fractures has a significant impact on various functional abilities.

Sixty-four patients with hip fracture and dementia were randomized to usual care or a multidiciplinary intervention consisting of staff education, care planning, and prevention of delirium and other postoperative complications. Experimental group had fewer complications, better mobility, and higher functional performance at 12 months than the usual care group, providing evidence that patients with dementia can benefit from multidisciplinary geriatric assessment and rehabilitation and should not be excluded from rehabilitation programs.
