Effective rescue of dystrophin improves cardiac function in dystrophin-deficient mice by a modified morpholino oligomer

The authors demonstrate restoration of dystrophin expression in the mdx mouse using antisense oligonucleotides conjugated to an arginine-rich cell-penetrating peptide. This is the first paper to achieve successful cardiac dystrophin expression following systemic injection of morpholino antisense oligonucleotides. Questions remain about how well the added peptide will be tolerated by the host immune system.

Sustained expression of microdystrophin was achieved following intramuscular injection of rAAV6 encoding a canine microdystrophin. Maintenance of expression was attained using a transient immunosuppressive regimen. These data suggest that, while an immune response to AAV capsid is likely to occur in dystrophic individuals, it can likely be prevented using transient immunosuppression.

The authors demonstrate the functionality of a microutrophin construct. This approach may be an important and viable alternative if dystrophin-based constructs elicit a cellular immune response in DMD patients, which seems likely. The endogenous, ubiquitous expression of utrophin increases the likelihood that a patient may develop immune tolerance of a utrophin-based transgene.
